Your attachments

Attach in PDF the documentation scanned in high resolution. We recommend using the Adobe Scan or Camscanner app.

Maximum weight: 30 mb

  1. Passport of the applicant of NIF
  2. Portugal Visa or Stamp of entry in the UE (optional)
  3. Proof of address outside Portugal
  4. In case of minors, passport of one of the parents and birth certificate must be enclosed.

Once the form has been sent, we will send you by e-mail a power of attorney that you will have to sign and send back to us in order to proceed with your application.

Documents accepted as proof of residence

  • The proof of address must be from the place where you reside, outside Portugal.
  • It must have your name and address.
  • It must be in one of the following languages: Portuguese, English, French, Spanish, Italian. Otherwise, a translation must be attached.
  • The following are accepted as proof of address: ID card, driver's license, utility bill, telephone bill, health insurance bill, bank statement, statement from government authority.
  • This voucher is for information purposes only and no documents will be sent to that address.
